What do you need to open your mailbox?
- Complete USPS Form 1583 provided at the store.
- Bring in the form along with two forms of identification:
- a. current driver’s license or state identification card
- b. armed forces, government, university, or recognized corporation identification
card - c. passport
- d. alien registration card or certificate of naturalization
- e. current lease, mortgage or Deed of Trust
- f. voter or vehicle registration card
- g. home or vehicle insurance registration card
A photocopy of your identification may be retained by the Kopy Kat Office when you open your
private mailbox, as per USPS requirements.
Cost
12-month cost breakdown. All costs associated with private mailbox must be paid in advance to receive one month free.
- Private mailboxes are $40.00 each month. One month free for the first year only if paid in full.
This includes unlimited small packages under 35 lbs. - Any package of 35lbs to 50 lbs. will be assessed at an additional $2.00 each.
- Packages from 50lbs to 100lbs. will be assessed at an additional $10.00 each.
- All packages 15 to 100 lbs. must be picked up no later than two days after you are called
excluding holidays. - Set-up fee includes one key, $35.00 with $10.00 for return of the key at the end of the rental.
- Total due upon approval is $515.00 plus tax if paid in full
- Three month minimum required would be $155.00
6-months cost breakdown. Three months advance costs associated with private mailbox must be paid in advance.
- Private mailboxes are $43.00 each month.
- This includes unlimited small packages under 35 lbs.
- Any package of 35lbs to 50 lbs. will be assessed at an additional $2.00 each.
- Packages from 50lbs to 100lbs. will be assessed at an additional $10.00 each.
- All packages 15 to 100 lbs. must be picked up no later than two days after you are called
excluding holidays. - Set-up fee includes one key, $35.00 with $10.00 for return of the key at the end of the rental.
- Total due upon approval is $258.00 plus tax if paid in full.
- For required 3 months payment upon approval would be $164.00

- This includes unlimited small packages under 35 lbs.
- Any package of 35lbs to 50 lbs. will be assessed at an additional $2.00 each.
- Packages from 50lbs to 100lbs. will be assessed at an additional $10.00 each.
- All packages 15 to 100 lbs. must be picked up no later than two days after you are called
